He's had lots of tests done and hospital seems to think there's a blockage. Anyone had experience of this or any advice (without using pipe cleaners!!)

have brother-man take some Saw Palmetto, Fennelgreek seed tablets, Zinc, and Magnesium. could throw in some Vitamin E+Selenium to augment the process.

maybe he should be given the number of a professional in the field because blockage can be quite serious.
i mean a normal man has millions of sperm cells
also, it could be something going on with his prostate gland, the gland that has to do with a mans "function"
definitely go and see as many doctors as it takes to find the problem.
he could just have an enlarged prostate, which would explain the difficulty in the sperm exiting, but…. better safe than sorry
If the tests have consistently shown he has zero sperm count then he is suffering from azoospermia. Many men with this condition can still ejaculate as normal, but there is no sperm in their semen.
Its entirely plausible he has a blockage of the epididymis, the passageway leading from the testes.
There is a surgery to correct this, but I believe it is extremely technical and complex. If you are trying to conceive, then there are of course artificial ways this can be done, like IVF.
