If you have already seen a doctor for the problem, it is now a pre-existing condition, and will not be covered under any plan for at least a year, if ever. Changing health insurers is tricky. Just make sure that you are not being treated for anything when you switch!

It should. The insurance I have through my job has an infertility benefit–it should apply to either the man or the women, whoever is having the issue. The coverage isn't that much but it would help a little.
However, just off the top of my head I don't think they can normally do much to help if the fertility issue is on the man's side. In vitro or other things like that typically get charged to the woman's insurance since the procedure itself isn't being done to the man.
